Health Minister Manuel Pizarro will visit the Portuguese Association for the Prevention of Diabetes (APDP) tomorrow. This visit coincides with the recent signing of the cooperation protocol that recognizes APDP as an integrated unit of the National Health Service (SNS).
This association provides support to people with diabetes through an integrated service, with multidisciplinary teams and focusing on education for greater independence and social inclusion of people with diabetes.
“What we advocate is a model of integrated intermediate care, between emergency care, complex hospitals and closer primary care. This model can be used for all chronic diseases,” said José Manuel Boaveda, President of APDP.
“It makes sense to create a model outside of hospitals and acute care, but one that allows support to be delivered more closely, while helping to get millions of people out of hospitals who can then be monitored in intermediate structures much closer to primary care.” APDP Chairman Highlights.
